TDAP VACCINATION: 7th Grade Immunization

All students entering the 7th grade must show proof of the Tdap vaccination by presenting an updated Mississippi Health Compliance Form 121. The Tdap vaccination protects against tetanus, diphtheria, and whooping cough (pertussis), a highly contagious disease that can be severe or fatal, especially in young children.

Parents may upload the Health Compliance Form 121 during online registration or provide the school with a copy of the Health Compliance Form 121 as proof of vaccination. The local Health Department or your private physician will provide this during your child’s vaccination.

Jones County Health Department in Ellisville will hold a special immunization walk-in clinic on June 23, 2023, from 8:30 am until 3:30 pm. Your child will be able to get up to date with school-required vaccinations. Parents should bring a copy of their child's immunization record.
